This dataset contains documentation related to a mixed-methods study on nurse-led transition clinics in the Netherlands, including the interview topic list, questionnaires and participant information letters used during data collection. These documents support transparency and reuse of the study design. Due to privacy and confidentiality considerations, the original qualitative and quantitative research data are not publicly available. Some parts of the questionnaire may be subject to copyright of the original authors.
