In this study, an intervention aiming to improve interprofessional collaboration, involving the social network, around citizens in multi-problem situations called "De Tafel Samen aan Zet" was investigated in the municipality of Harderwijk. Process and outcomes have been continuously monitored for two years (2018-2020) in a responsive evaluation.
